Whole system design

Give each question a score and once completed document some observations and examples for each aspect 0 – 24 Poor (significant risk), 25 – 49 Fair (lesser/lower risk, improvement required) ,50 – 74 Good (shortfalls identified/need to be addressed) 75 – 100 Excellent (no action required)​

Are we making a place that helps everyone live life well?

How are we working as a whole, place-based system?

Do we understand need and capacity as a whole?

Is the whole system coherent? Do we have a plan as a whole system?

Are we putting social and environmental justice at the heart of the system?

Can we shift the dial to building on strengths? Developing an enabling, technology-supported self-help system, achieving prevention and early intervention?

Are we learning as a whole system?

Can we work systematically at a strategic, place, , community and individual level?
